S2 Q&A: The form asks Initial, Transfer, or Change of Status - which am I?

Transfer if you hold F-1 now, including on OPT - your SEVIS record moves over. Change of Status if you're in the US on another status (H-1B, L-1, B-2...) and will file to become F-1.
